$10,000 Sign on BonusGENERAL SUMMARY:Under the general supervision of a nurse manager or other nursing leader, performs diversified clinical duties by providing nursing care to acutely or chronically ill patients. -The Transplant Specialty RN is a member of the Multi-Organ Multidisciplinary team, functioning as a practitioner, patient/family educator, consultant/resource to coordinate the post-transplant phases of care for the kidney and liver transplant recipient within an outpatient setting.ESSENTIAL DUTIES:1. Provides specialized nursing care to patients with illnesses, combined medical problems or attitudes that require adaptation of established nursing procedures2. Acts as liaison between the patient and family, physicians and other members of the interdisciplinary team3. Administers prescribed IV medications, blood and blood components and intravenous fluids and monitors patient response to drug treatments4. Provides wound management, nutritional guidance, and psychological support to patients, families and to the professional staff5. Interprets and implements policies and procedures set forth by nursing and hospital administration6. Functions as a clinical and community resource, educator, and consultant.7. Able to coordinate and provide care for patients based on detailed treatment protocols initiated by physicians without need of direct physician supervision.8. Duties as assigned based on specific specialty and/or service line.9. Coordinates care for the post transplant/nephrology patient in an outpatient setting including:

  • Assesses and evaluates post-transplant patients in clinic or via telehealth
    • Administers Flu shots, EPO (Procrit)/Aranesp injections
    • Performs wound care for incisional wounds and dressing changes for PICC lines and Perma Caths
    • Removes incisional staples and sutures
    • Reviews and triages patient lab work
    • Collaborates with other team members as needed (i.e. dietitian, social worker, financial coordinator, pharmacist, nurse practitioner, HLA laboratory)
    • Assists with patient prescription assistance program and maintenance of prescription assistance log
    • Updates referring physicians on patient status
    • Triages all calls regarding patient problems and concerns and discusses appropriate situations with the physician
    • Manages patient discharge medications, new prescriptions and medication refill requests
    • Arranges for medication administration given to patients in the Beaumont Infusion Center
    • Schedules biopsies (renal/liver) and submits the biopsy procedure orders
    • Arranges and obtains orders for Pentamidine treatments.
    • Arranges renal/liver ultrasounds, Interventional Radiology procedures
    • Schedules and obtains orders for various tests and procedures, (Rituxan, Thymoglobulin, Iron infusions, Plasmapheresis, Solumedrol, IVIG)
    • Utilizes EPIC/Phoenix for documentation/communication to team
    • Conducts education for scheduled pre-living donor transplant candidate/family
    • Provides education for the post-transplant patient prior to discharge (inpatient setting)
    • Provides education for patients learning self-administration of EPO (Procrit)/Aranesp
      • Assists with and participates in patient/family educational seminars and support group meetings
      • Completes patient's insurance and disability forms
      • Completes federally required forms. - Responsible for completion of the post-transplant follow-up and Transplant Malignancy forms electronically in TIEDI
      • Participates in department/hospital committees (e.g. QAPI, CMS, Clinic Operations/ staff meetings, etc.).
        • Responsible for agenda/minutes and attends Multi Organ Transplant Team meetings (Grand Rounds).
        • Assists in the management of the annual continuing medical education application for Transplant Grand Rounds.
        • Attends weekly kidney and liver selection committee meetings
          • Updates transplant protocols and develops and maintains educational materials
          • Assists in the development, implementation, data collection, and follow-up of transplant research protocols
          • Involvement and knowledge of the Transplant QAPI Plan. -
          • Participates in QA Projects - medication reconciliation, chart reviews, medication cabinet review, hand washing audit, discharge teaching evaluation
          • Helps plan and attend various community education/celebration events (donor tea) NKF fundraisers, donor awareness week, etc.)STANDARD QUALIFICATIONS:A. Education / Training: --- Equivalent to a Bachelor of Science degree in nursing from an NLN-approved school of nursing.B. Work Experience: --- Three years of acute nursing experience required. 2 years nursing experience required in Transplant, Nephrology, Hepatology -preferred. Outpatient nursing experience preferred.C.
