Unit Based APP-Nurse Practitioner for Medical Progressive Units 4 Center and 6 North

GENERAL SUMMARY:

Under the general guidance of the medical director and using principles of patient and family centered care, the unit-based nurse practitioner will support nursing staff and patient safety through education, and consultation of patients with diseases and adverse health conditions following the written guidelines approved by the department and medical staff rules and regulations.

ESSENTIAL DUTIES:

* Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies.
* Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up.
* Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols.
* Determines eligibility of patients for entry into clinical trials based on findings.
* Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care.
* Interprets and evaluates findings of studies/tests.
* Relays appropriate information regarding patient care to the collaborating physician.
* Administers therapeutic measures and obtains specimens as prescribed.
* Evaluates the quality of care provided and recommends changes for improvement.
* Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the client.
* Dictates follow-up letters to referring physicians with summaries of treatment, response, and plan.
* Maintains provision of preventive health services (e.g. screening, risks assessment, immunizations, PAP smears, self-breast exam).
* Dictates or writes patient history, admissions, care plans, progress, and discharge notes.
* Interacts with program assistants and data managers for optimal patient care.
* Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care.
* Conducts or participates in daily multidisciplinary rounds and ensures appropriate and quality care for assigned patient.
* Provides on-call coverage.
* Performs patient triage and determines the urgency of physician's evaluation.
* Provides symptom management for side effects related to oncologic treatment.
* Supports nurse driven practice model and acts as unit based clinical resource and consultant for nursing staff.
* Assesses the physical and psychosocial status of patients based upon health history, physical examination, and diagnostic studies.
* Recognizes deviations from normal in the physical assessment. Works in consultation and collaboration with the multidisciplinary team in the development of an ongoing plan of care for select patients on the unit, including patients with an acute or immediate need in collaboration with the physician.
* Assist with throughput initiatives, including attending unit-based rounds, reviewing the appropriate level of care, and potential for transfer or discharge.
* Assist nursing to facilitate discharge and post-discharge teaching in accordance with written practice protocols.
* In collaboration with the nursing team, relays appropriate information regarding patient care to the physician and multidisciplinary care team
* Complete documentation and update the problem list for acute problems being addressed by APP
* In collaboration with the nursing team, relays appropriate information regarding patient care to the physician and multidisciplinary care team
* Communicates plan of care and patient status with patients and families and assist with coping mechanisms
* In an acute situation, performs patient triage and determines need for activation of the rapid response team and physician notification

STANDARD QUALIFICATIONS

* Education / Training:
* Master's Degree in Nursing from an Acute Care Nurse Practitioner accredited program/institution
* Work Experience:
* 1 year of clinical experience as an RN required and 1 year of experience as an Advanced Practice nurse in a specialty clinical setting preferred


* Certification, Licensure, Registration:
* State of Michigan Nurse Practitioner license required
* AHA/ARC BLS certification required
* DEA required
* Certification as ACUTE CARE Nurse Practitioner through ANCC, or AANP
* Current ACLS Certification within 3 months of hire

D. Other Qualifications:

* Demonstrates effective written and oral communication skills consistent with mission and values.
* Demonstrates interpersonal skills necessary to interact with patients in situations requiring sensitivity and tact.
* Demonstrates strong health assessment skills.
