Assessments, planning, implementation, evaluation, and discharge needs of patients and families are addressed. Core Concepts of Patient- and Family-Centered Care are applied in practice. Quality care that meets or exceeds expectations is provided. Assistance is offered in a manner that considers special needs, age-related, and cultural needs. Knowledge of technical procedures and proper use of equipment is demonstrated. Documentation in the patient record is timely, accurate, and concise. Continuing personal education, mentoring peers, and patient education are emphasized. Current health system initiatives and evidence-based practices are incorporated into care. Self-appraisals including professional goals are completed annually. Participation in department-based activities, staff meetings, and committees is encouraged. Active participation in handoffs of care is practiced, including patient and family involvement. Understanding and embracing cultural traditions of the community served is essential.
