Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date:
4/20/2026 About the Position Provide direct social work services to
pediatric/adolescent patients and families Perform discharge
planning and case management in an acute inpatient setting
Collaborate with interdisciplinary teams, school staff, and
families Connect patients with community resources and support
services Ensure effective care coordination and positive patient
outcomes Requirements Master’s Degree in Social Work (MSW) from an
accredited program Active Michigan Social Work License (LMSW or
LLMSW) Minimum 5 years of case management experience At least 2
years of recent acute inpatient case management experience (strict
requirement) Strong discharge planning experience
